ROCKART CUSTOM DECAL QUOTE REQUEST FORM

This form contains the exact questions we would ask you if you phoned us with a custom decal quote request. Please fill it out as best as you can -- we will call or email you if we have any additional questions. Once we receive this form, and you have uploaded any artwork necessary, we will prepare and send you a quote for producing your decal. Please allow two to four business days for quote preparation.

10) Artwork:

	Decal is all text, or uses a standard logo that Rockart already has.
	We will be supplying Rockart with hi-resolution artwork in vector format (as in Adobe Illustrator .ai or .eps files, with type converted to outlines and colors designated as spot colors with Pantone PMS values).
	We will be supplying Rockart with artwork, but it is in .tiff, .jpg, PhotoShop .eps or .psd or another raster format and we will need Rockart to convert this to a screen-printing-friendly format. ($45 per hour art charges will apply.)
	We have a rough sketch of artwork that we want Rockart to create for us. ($45 per hour art charges will apply.) You can fax your sketch to us at 480-832-6390.
